Introduction

These terms and conditions govern the provision of podiatric and healthcare services ("Services") by Sydenham Podiatry ("Provider") to the patient ("Patient"). By using our Services, you agree to these terms and conditions.

Provider Qualifications

The Provider is qualified to provide podiatric and healthcare services and is registered with the relevant regulatory authorities in Australia.

Scope of Services

The Provider agrees to provide the Services to the Patient with due care and skill. The Provider will determine the scope and nature of the Services required by the Patient in accordance with professional standards and ethical guidelines.

Fees and Payments

The Patient agrees to pay the Provider for the Services provided, in accordance with the fees set out by the Provider. The fees may be subject to change, and the Patient will be informed of any changes prior to the provision of Services. The Patient is responsible for any additional costs associated with the provision of Services, such as diagnostic tests, imaging, or specialist consultations.

Appointment Cancellation and Rescheduling

The Patient may cancel or reschedule an appointment by providing the Provider with reasonable notice. The Provider may charge a fee for cancellations or rescheduling with less than 24 hours' notice.

Patient Information and Privacy

The Patient agrees to provide accurate and complete personal and medical information to the Provider. The Provider will use this information only for the purpose of providing the Services and will take reasonable steps to protect the privacy and confidentiality of the Patient's information.

Consent to Treatment

The Patient consents to the provision of the Services by the Provider. The Provider will explain the nature and risks of the Services to the Patient, and the Patient will have the opportunity to ask questions and seek clarification. The Patient has the right to withdraw consent at any time.

Complaints and Disputes

The Patient may raise any concerns or complaints with the Provider. The Provider will take all complaints seriously and will investigate the complaint and respond to the Patient within a reasonable time period. If the Patient is not satisfied with the Provider's response, the Patient may raise the matter with the relevant regulatory authority.

Liability and Indemnity

The Provider is not liable for any loss, damage, or injury suffered by the Patient as a result of the Services, except to the extent that such liability cannot be excluded under Australian law. The Patient agrees to indemnify the Provider against any claims or actions brought against the Provider as a result of the Patient's breach of these terms and conditions.

Governing Law

These terms and conditions are governed by the laws of the state of Victoria, Australia, and any disputes will be resolved in accordance with the laws of that state.

Amendments

These terms and conditions may be amended from time to time at the discretion of the Provider. The Patient will be notified of any amendments prior to the provision of Services.

Entire Agreement

These terms and conditions constitute the entire agreement between the Provider and the Patient and supersede any prior agreements or understandings between the parties.
